America has finally woken up to ISO 9000, TQM is not enough

HQS SURVEILLANCE AUDIT
A surveillance audit was conducted on November 19-21, 2002, by the NASA registrar, NQA, Inc. The audit resulted in one nonconformance and three observations. The findings were as followings:
Nonconformance:
The ISO 9001 Program Office did not ensure that an independent internal auditor audited the Headquarters QMS internal audit process.
Observations:
The draft SMA Requirements Model did not include a description, performance goals and measurements for all key sub-elements. Sub-element examples include SMA Policy Development, Safety Surveys, Work Site Hazard Analyses, and Program Office Safety.
Code Q does not have a single data store to analyze audit findings across all SMA-like audits.
Observation 3 was a continuation from the May registration audit and concerns the review of IPAO data analysis results by the PMC.
